Trike inspection.Help!

Ive just finished my trike and its passed an Mot. I now need to book it in for the MVSA test but I'm unsure what paperwork I need to fill in. I have the MSVA1 form, from VOSA, a V55/5 from DVLA and a V627/1 from DVLA. What form(s) do I need to send to the MVSA test station along with my fee? And once its been tested and hopefully passed what do I do next to get it on the road?

Any help would be greatly appreciated.

You can do one of two things
If you have a log book for the donor vehicle then send it to swansea with the change of wheelplan filled in
or just phone your local DVLA office which you'll find in yellow pages under "DRIVERS"
either way you'll get all the forms and info you need
